INFORMATION FOR COLLEGE STUDENTS
The DC 33 Aetna HMO plan is a national network within the United States; therefore, individuals can also access care whenever they are away.
For those students who will be temporarily away at college and who want to continue to use the DC 33 HMO plan for non-emergency visits and referrals, they need to follow these two steps:

Call Aetna Customer Service at 1-800-370-4526.

Choose a new primary care physician in that area.
Once the student returns home, they can contact Aetna to switch back to their JFK primary physician.
It is important to note that the Emergency Room and Urgent Care services are still accessible wherever the student is currently located.


2 FORMS OF I.D
We want to remind our patients of the importance of having two forms of ID when visiting JFK Medical Center for scheduled or unscheduled appointments.
First, you MUST BRING YOUR HEALTH INSURANCE CARD.
In addition, you must have one of the following acceptable photo IDs:

City ID (with photo)

Driver's license (with photo)

State-issued ID card (with photo)
-
School ID (with photo)

NO CO-PAY for medical and lab services for active members of AFSCME District Council 33 and their dependents!
